Severe Calcinosis in Limited Cutaneous Systemic Sclerosis

چکیده
A 56-year-old woman presented with long-standing pain in both knees. She had been suffering from Raynaud’s phenomenon for 10 years. A physical examination showed hard subcutaneous nodules on the extensor surfaces of both knees and elbows (Picture 1: left elbow and right knee) with sclerodactyly and telangieclagia in both hands. The Creactive protein level was normal (0.08 mg/dL, normal < 0.3), anticentromere antibody was positive (100.9 U/mL, normal <7.0) and other autoantibodies were negative. A diagnosis of limited cutaneous systemic sclerosis (lcSSc) was made. Posteroanterior radiographs revealed severe calcinosis in the left elbow and right knee (Picture 2). Calcinosis mainly consists of cutaneous deposits of hydroxyapatite and is found in about 40% of patients with SSc (1). The lesions are commonly located in pressure areas, such as extensor surfaces. Physicians should be alert to presence of calcinosis on a physical examination of patients with SSc because it can lead to significant disability, ulceration of the skin, and infection.

Dystrophic Calcinosis in a Patient with Scleroderma: Imaging Findings and Literature Review
Subcutaneous calcinosis consists of abnormal calcium deposition in soft tissues and is one of the complications of scleroderma, especially in limited systemic sclerosis. It is one of the criteria of CREST syndrome (calcinosis cutis, Raynaud phenomenon, esophageal dysfunction, sclerodactyly, and telangiectasia). A 44-year-old woman with systemic scleroderma for 20 years was presented with pain ...

Calcinosis is a well-recognized manifestation of systemic sclerosis. Paraspinal or intraspinal calcinosis is rare, with reports of calcinosis involving the cervical, thoracic and lumbar separately, but not together. We now report a case of limited cutaneous scleroderma with extensive paraspinal calcinosis of the cervical and lumbar spine.

OBJECTIVES To evaluate the effect of minocycline as treatment for cutaneous calcinosis in limited cutaneous systemic sclerosis (lcSSc). METHODS Patients with lcSSc who had cutaneous calcinosis causing pain or ulceration, or both, were prescribed minocycline 50 or 100 mg daily regularly in an open label manner between November 1994 and April 2000. At routine clinical follow up the appearance o...

متن کاملTreatment options for the cutaneous manifestations of systemic sclerosis.
Systemic Sclerosis is a multisystem disorder with vascular instability as a clinical hallmark. Treatment currently consists of recognition and management of end-organ damage. Dermatologists can assist in the management of these patients by facilitating early diagnosis, and treating cutaneous manifestations such as Raynaud's phenomenon, cutaneous calcinosis, and digital ulceration. New potential...
